Program Options
After you have been approved to join the AzureWell Practitioner Program, you can choose either or both options below to suit your needs.

(A) Client Express with a Promo Code: By using Azure Standard's "Promo Code", your clients can directly purchase all the AzureWell branded items as well as items from the abundance of healthy foods and products available at Azure Standard. This allows you to potentially receive a significant commission opportunity. (Comprehensive details on this mechanism are shared below).
 

1. Client's Discount: Your client receives the discount you specify with as many redemptions as you specify.

2. Your Commission: You receive a 35% commission on all AzureWell brand purchases your client makes going forward (not just when the Promo Code is used).

(B) Stock The Products: Purchase AzureWell products from Azure Standard at a wholesale rate.

1. Pricing: With a wholesale account, you will have access to pricing comparable to that of a retail store. This allows you to mark up the price of the items as you see fit.

2. Staying Active in the Program: To maintain participation in this AzureWell Practitioner Program (even when choosing the option to Stock the Products primarily), you will need at least 2 new customers activated under your Practitioner Account each year. They become activated under your account when they use your Client Express Promo Code at Azure Standard check-out. By actively recruiting at least 2 new customers per year, this simply lets us know that you are still active in the Practitioner Program. (More details about our Participation Criteria are included below).

3. If you prefer not to ever use Promo Codes and wish to only sell from your clinic and/or drop ship from Azure Standard, we suggest the alternative Wholesale Account option below.

4. Alternate Wholesale Account option: While we believe our Practitioner Program has great advantages, If you choose to simply stock AzureWell brand products (and/or any other product distributed by Azure Standard) in your clinic, you can operate with a simple wholesale account at Azure. Essentially, by choosing to not ever use a Client Express Promo Code, you are choosing not to participate in the Practitioner Program, but instead are choosing to be a regular wholesale business account Such an account requires that you maintain a minimum cumulative purchase amount from Azure Standard of $10,000 annually.

 

Practitioner Compliance Disclaimer
Health professionals are subject to different laws and requirements depending on licensure, geographic location, and other circumstances unique to the health professional. By enrolling and participating in the Practitioner Program, each health professional acknowledges and agrees that (a) he/she is solely responsible for assessing and complying with his/her professional obligations under applicable laws, ethical codes of conduct, and other binding guidance, including laws governing professional licensure and certification, patient relationships, other industry relationships, and health information privacy and security; (b) he/she is not precluded legally or ethically from participation in the Practitioner Program; and (c) he/she holds harmless Azure Standard and its affiliates, subsidiaries, owners, directors, officers, and employees from all losses and costs of any kind or nature (including attorney's fees) associated with the health professional'sparticipation in the Practitioner Program.

Participation Criteria
To maintain active status in the Practitioner Program, it is required to have a minimum of two new clients use your Promo Code annually. Should you not meet the yearly minimum, we will send reminders and check in to understand your desired direction with the Program. If you decide to stop participating in the AzureWell Practitioner Program, it's straightforward - simply contact us via email or phone 971-200-8352 to deactivate your Practitioner Program Account.

AzureWell's Practitioner Program is an extensive toolkit empowering health professionals to shape a well-rounded wellness journey delivering innovative solutions to address all aspects of your clients' well being. While our aim is to uphold the Program's longevity and ensure you reap its advantages, we must clarify that AzureWell retains the ultimate right to change or conclude the program in its entirety or for a specific Practitioner at its discretion, without prior notice, and for any or no given reason.

Online Sales
Permission is granted to the health professionals in our AzureWell Practitioner Program to sell Azure products in their own brick-and-mortar clinic(s) and store(s) and on their own digital platforms. These digital platforms (such as websites and mobile apps) must be under the control of the practitioner and ithe URL(s) owned and
managed by the practitioner.

Sales through any third-party platform (such as Amazon, eBay, Walmart, Marketplace, among others) is not allowed.

At AzureWell, we believe in empowering practit oner prosperity to the depths of their expertise. AzureWell practitioners can now e evate their earning potential beyond the confines of traditional dietary supplement recommendations.

Every item sent to a client (from the vast array of all that AzureWell and Azure Standard have to offer), represents a commission opportunity.* This includes supplements, health & body care products, non-foods, and groceries - from grains, fresh produce, and nuts to frozen meats, beverages, and more.

It's a holistic approach that recognizes and rewards the breadth of a practitioner's mastery, allowing practitioners to monetize their comprehensive health and wellness knowledge. With AzureWell, practitioners don't Just share their wisdom; they truly benefit from the full spectrum of their client's journey toward optimal well being.

To enable us to offer both AzureWell products and food & lifestyle products through this Practitioner Program, products within the AzureWell Practitioner Program will be administered and fulfilled through Azure Standard.

Managing Product Recommendations
With the increase in tele med appointments, and the benefits of recommending products beyond what you wish what you wish to stock in your clinic, our Client Express feature is a game-changer  Simply give your client your special Promo Code They get a discount. you get an on-going commission. (Details below). Of course you can choose to stock the products too if you wish.

You will need to name your AzureWell Promo Code before it is activated by Azure Standard. (Example: DRSMITH10 for a 10% discount to the client)

Once you have named your unique personalized Promo Code, available options for configuring the Promo Code
allow you to:
 

1. Set your discount to clients for AzureWell brand items and,

2. Choose how many times a client can use the code to get that discount.

We strongly recommend that you offer a discount with this Promo Code to motivate your clients to actually use the Promo Code. (The client only has to use the code one time for you to start receiving commissions.) Choose a discount for your clients that will be of value.  When the discount is entered in Azure's systems a% off
greater than your return will not be allowed.

The Promo Code setup gives you the leeway to decide If It's for a single use or can be redeemed multiple times. (The client needs to enter the promo code at check out each time they want the discount.)

From the first time your client uses your special Promo Code on an Azure Standard order, their account will be
connected to your Practitioner account.

Beyond AzureWell Branded Products
As described above, the AzureWell Practitioner Program includes a commission of 35% for all AzureWell branded products. But, there's more! You wlii aiso receive a commission on aii other Azure Standard items (excluding gift cards).

We designed a tiered system for this part of the Program to reward practitioners based on their sales performance for Azure Standard products (excluding AzureWell items). This program aims to encourage practitioners to actively promote and sell Azure Standard products to their clients. The% return for Azure Standard items is different from those for the AzureWell branded items.

Commission Structure
Practitioners will earn commissions based on their sales volume, determined by their 6 month rolling sales average of Azure Standard and AzureWell items to clients who have used the practitioner's Client Express Promo Code.

The levels are:

1. Up to $5,000 in sales every 6 months

2. $5,001 to $50,000 in sales every 6 months

3. $50,001 and above in sales every 6 months

The following commission levels apply:

1. Level 1: 0.5% return to the Practitioner

2. Level 2: 1% return to the Practitioner

3. Level 3: 2% return to the Practitioner

Level Evaluation
This rolling evaluation allows practitioners to move up or down in the commission levels based on their consistent performance. The level adjustments will be made on a monthly basis.

Understanding Promo Codes for Practitioners
From the very first time your client utilizes your Promo Code, their Azure Standard account is linked to you. Within your Azure Standard account, you can view this client's total AzureWell purchase totals and the associated Promo Code values In your Practitioner Program ledger. This ledger clearly displays your earnings versus the discounts provided to the client.

 

Pay-Out Options
The commission % from AzureWell product sales are credited to your Azure Standard account. You have three choices to handle these credits:

 

Net Balance Account:
Pay the net balance on your account as of the end of the month, or receive a check if it's in your favor. For this, you'll need to provide a W-9 form, allowing us to complete a yearly 1099 form for the IRS on your behalf. Send your W-9 form to info@azurellvingwell.com.

Monthly Check:
Receive a monthly check payment for your Practitioner Program earnings. For this, you'll need to provide a W-9 form, allowing us to complete a yearly 1099 form for the IRS on your behalf. Send your W-9 form to info@azurelivingwell.com.

 

Azure Credit Only:
Use the credit exclusively for Azure Standard product purchases (no cash payouts for earnings from the Practitioner Program).

 

Program Eligibility
We are eager to extend the Practitioner Program opportunity to health professionals from varied backgrounds. This program is tailored for those health professionals whose role encompasses consulting clients on nutrition and holistic wellness.

 

To be considered for the Practitioner Program, the applicant needs to meet at least one of the following criteria:
 

• Hold a license, certificate, or other relevant educational credentials in thehealthfieldor be actively

• Have engaged inhealth-focused continuing education sessions in the past two years.

• Receive a recommendationas an internunder anaccredited practitioner.

At this time, we are not extending the Practitioner Program to health professionals participating in or accepting reimbursement from any public payor programs (such as Medicare or Medicaid). By enrolling and participating In the Practitioner Program, you verify that you understand that this program is not intended for those accepting the above public payor programs and agree to hold Azure Standard and all its affiliations harmless from any responsibility of any nature associated with your participation in any payor program. Should you make a change to your policy concerning public payor programs, you take full responsibility for notifying Azure should this Practitioner Program status need to change.

Azure Standard reserves the right at all times to determine Practitioner Program eligibility and participation,Including the ability to decline enrollment and terminate participation of any health professional, all in its sole discretion.
